Summer Application
The
Custom Skills Special Application
To
enroll in a summer program, a parent or guardian must fill
out the registration form on-line and send $100 day camp,
and $150 overnight camp non-refundable deposit to Ferdie's
Soccer Magic Academy via snail mail to reserve your spot
at camp. You can also fill out the form, print it and mail
with your deposit. Deposit is non refundable. Applications
received after May 31 must include full payment. Tuition
balance is due no later than June 30 2004.

Enrollment
for the each camp is limited to the first 100 applications
per session. Beyond that number students will be accepted
on a stand by basis. Unadmitted applications will be fully
refunded. Should Ferdie's Soccer Magic Academy be cancelled
for any reason, all applicants will be fully refunded.
Online
applications will receive a short e-note of thanks. This
note means your application went through successfully. Go
ahead and send in your deposit to secure your spot. Receipt
of your deposit saves your spot. FSM Academy will send each
registered camper a written confirmation with a complete
list of what to bring to camp, check in and out information,
as well as directions to camp. This will happen around end
of May.
Please
note that receipt of deposit secures your enrollment.
Camp
Tuition also covers: Accident insurance, meals, special
awards and certificate of completion. Overnight tuition
includes lodging.
All
campers must have health insurance, An immunization record,
and a physical to participate in Camp. This is a must so
please prepare for it.
